How do they still have in a war-torn country and really poor countries where famine is everywhere?

I can understand why they have its natural and probably the only pleasure they have and they probably do not know of any better conditions as they do not have T.Vs newspapers etc. What I don't understand is how they get pregnant when they are so skinny and under nourished and how the babies survive and grow up as I know a lot of the women do not survive giving birth so they are not around to feed the babies.
